New Patient Information

Synergy Health Care is dedicated to providing you the best possible care. With a team of two doctors evaluating your history and exam from a comprehensive traditional Osteopathic approach addressing body, mind and spirit; we strive to determine the causes of your health concerns and create a plan to eliminate the problems by treating the cause.
When you inquire about becoming a patient an appointment will be scheduled for a call with our intake coordinator. You will be asked the questions on the intake form ( Intake Form download) and an appointment will be set for you.
Your first visit will be 1 1/2 – 2 hours long. Follow-up visits are 45 -60 minutes long depending on the services. To get the most out of your first visit please download and complete all of the new patient forms. You will need to bring those to your first appointment. Please take the time to complete them in detail. Included is a 2 week sleep log and a 3 day diet history. The more attention you give to completing the history and review of symptoms the better we are able to help you. In addition bring along copies of test results or reports that you have.

What to Expect at your first appointment

At your first appointment we will review your history and the forms you have completed, asking questions and getting additional details from you. Wear loose comfortable clothing. You will be asked to change into a gown and a physical exam will be completed. Your physical examination will include an osteopathic structural examination which looks at not only your neck and back, but your arms, legs and feet, the tension in your fascia (a type of connective tissue) muscles, organs, and the cranial mechanism. A plan will be developed for you based on the information we collect from your history and examination. In most cases lab tests will be ordered, sometimes X-rays, MRI or ultrasound is needed as well. You will then be scheduled for a lab review appointment and Osteopathic treatment as indicated by the findings of your structural and physical examination.

Insurance Coverage

Your insurance is a contract between you and the insurance company. Synergy Health Care is not contracted with any insurance company, so all care provided is under your out of network benefit package. You are responsible for the cost of services provided and payment is expected at time of service.
Your insurance company is not obligated to pay for services you receive, they may determine that they are not medically necessary or are non-covered services.
The intake coordinator will review SHC payment policies and fee schedule with you. Typically new patient appointments are booked out 6-8 weeks, but on occasion there will be an earlier appointment available, you can also be put on a waiting list in the event an earlier option opens up.
Prolotherapy is a non-covered service in the majority of cases because most insurance companies consider it experimental and not medically necessary.
Osteopathic treatment is a manipulation therapy. Many insurance policies have limitations on the number of visits per year and include in that total: osteopathic treatment, chiropractic treatment and physical therapy.
